How to Refer Patients to Lakes Region Retina
How to Refer a Patient
You may refer patients to Lakes Region Retina using any of the following methods:
Phone Referral: Call our office at (218) 297-5344 to schedule an appointment while the patient is in your office, or to discuss the case directly with the physician.
Faxed Referral: Fax referral information to (218) 297-5363
Please include: patient name and date of birth, contact information, insurance details, reason for referral, and relevant records or imaging.
Secure Electronic Referral: If your practice uses a secure messaging or referral platform, referrals may be sent directly to Lakes Region Retina through that system. We will confirm receipt and contact your patient promptly.
Our staff will contact the patient to schedule the visit, verify insurance, and provide clear instructions before the appointment.
Insurance, Referrals, and Authorizations
Lakes Region Retina participates with Medicare and many private insurance plans. We assist patients in understanding referral and pre-authorization requirements. Referral forms or authorizations required by a health plan should be initiated through the referring office when possible.

Urgent and Same Day Referrals
We make every effort to see urgent cases in a timely and medically appropriate manner, including:
– Suspected or confirmed retinal tears or detachments
– New onset of flashes and/or floaters
– Sudden or significant vision loss
– Acute macular disease (e.g., new sub retinal hemorrhage, suspected wet AMD)
– Postoperative retinal complications
For urgent referrals, please call our office directly at (218) 297-5344 so we can prioritize the patient appropriately and coordinate with your office.
What Your Patients Can Expect
At their visit, patients can expect a thorough retina-focused evaluation, explanation of findings, discussion of treatment options, and an efficient experience designed to minimize waiting. We emphasize education and reassurance, especially for chronic conditions that may require ongoing care.
Communication Back to Your Office
After each consultation or treatment visit, we will send a summary of findings, diagnosis, and treatment plan. We will outline follow-up schedules, indicate any requested co-management, and notify you promptly of any significant changes in the patient’s condition.
