We’re glad you’re here. Our team is committed to making your visit comfortable, clear, and well supported from the moment you arrive. Here you’ll find helpful information for new patients, including what to expect at your first appointment, forms you can complete ahead of time, and answers to common questions. Whether you are coming on your own or with a caregiver, our goal is to help you feel prepared and confident in your care.
New Patient Forms
To help your first visit go smoothly, we encourage you to complete your intake forms ahead of time. These forms provide our team with the information needed to understand your health history and begin planning your care.
WHAT TO BRING:
- Your completed patient registration and medical history forms
- Insurance card(s)
- Photo identification
- Referral or authorization form (if required by your insurance)
- A list of your current medications, including eye drops
- Your current glasses or contact lenses, if applicable
- Any previous eye records or imaging if referred by another provider

What to Expect at Your Visit
Your initial visit will include a comprehensive examination and may involve diagnostic testing such as Optical Coherence Tomography (OCT), retinal photography, or fluorescein angiography (FA), depending on your condition. Please plan for your visit to take approximately 1.5 to 2 hours. Your eyes will be dilated during the examination, which may temporarily affect your vision. We recommend arranging for someone to drive you home if possible.
ONGOING CARE AND COMMUNICATION
If you require follow-up visits or ongoing treatment, we will work closely with you and your referring physician to coordinate care. Our staff will explain your treatment plan and answer any questions you may have before you leave the office.
Care, Coverage, and Support
We want every visit to feel safe and well supported. Retina exams often include dilation, which can make driving difficult, so we encourage patients to bring a caregiver, family member, or friend whenever possible. Below you’ll also find information about insurance and billing to help you prepare for your appointment.
CAREGIVER INFORMATION
Because dilation may temporarily affect your vision, having someone with you can make check-in, travel, and follow-up much easier. Caregivers, drivers, and family members are always welcome to accompany you during your appointment.
INSURANCE INFORMATION
Lakes Region Retina accepts a wide range of insurance plans to make care as accessible as possible. Please contact LRR in advance to confirm whether your insurance carrier is accepted.
Billing and Payment
Our secure online portal allows you to view statements, make payments, and manage your billing information at your convenience. If you have questions about a charge, need help navigating the portal, or prefer to handle payment by phone, our team is always happy to assist.
